Formal Evaluation Package

Includes comprehensive evaluation and a detailed write-up of the report.
Price: $500

Consultation Session

30-Minute Session
- Price: $165 per session
30-Minute Zoom Session
- Price: $150 per session

Treatment Sessions

60-Minute Session
- Price: $200 per session
45-Minute Session
- Price: $175 per session

All meetings are done either in the client's home or via Zoom

Payment Policy

Payment is due at the time of service.
I accept cash and checks
A Super Bill will be provided for each session, allowing you to submit for reimbursement through your insurance provider.

Cancellation Policy

Please provide at least 24 hours notice if you need to cancel or reschedule your appointment.
Cancellations with less than 24 hours notice may be subject to a cancellation fee.
